About Position: The primary responsibility of the Certified Nursing Assistant is to assist patients and perform other tasks as assigned. Also, to support patient care in all aspects of Adult Day Center, including driving, as needed.

  • Schedule: Full Time Monday – Friday: 8:30am - 5pm
  • Required Travel: None

Duties/ Responsibilities – 

  • Provide basic nursing care to assigned participants, including tasks such as, but not limited to: toileting, pressure relief and skin care.
  • Support care provided in rehabilitative therapies as directed by licensed therapists (OT, PT and ST).
  • Assists in delivery of dietary services in the Adult Day Rooms.
  • Provide transportation of passengers to and from their destinations as needed, and assist in patient care during transit and off-site medical appointments.
  • Safe transfer techniques and ambulation assistance.
  • Performs vital checks and other support as assigned in the clinic.
  • Provide operational support in day center, such as laundry & stocking inventory.
  • Monitor participants who are at risk of wandering.
  • Facilitate recreational activities.
  • Engage and support participants in activities.
  • Communicates effectively with participants, families, staff, agencies and other departments within PHS.
  • Documents assistance, observations, services and patient activities on appropriate forms.
  • Communicates any changes seen in participant condition to team members.
  • Assigned to a panel of participants and serves as a member of the IDT Team. Attends staff meetings and takes part in participant care planning.
  • If the CNA speaks fluently in a language other than English, the CNA provides non-medical translation services for the participant and caregivers as needed.

Qualifications – 

Education: High school diploma or equivalent. Successful completion of Nurse's Aide, Home Health Aid or equivalent training course.

Required: Must be able to work effectively in a team environment.

Must be able to provide assistance and services to people of all ages, including geriatrics.

Must meet a standardized set of competencies for the specific position description established by PHSC and approved by CMS before working independently.

Preferred: One year of experience with the frail or elderly preferred.

Current/valid License: Certified Nursing Assistant I or certified within 90 days of employment. Upon employment, must become trained in CPR and First Aid.

Immunizations: Be medically cleared for communicable diseases and have all immunizations up-to-date prior to beginning employment.

Pay Range - $15.15 - $20.36
