About Position: The Farmworker Health Outreach Specialist acts under the direction, guidance and supervision of the Farmworker Health Outreach Manager.  This position is responsible for completing and documentation of basic health assessments, referrals, and follow ups to migrant, H-2A, seasonal, and retired farmworkers.  Additionally, outreach activities, case management, and health education are core expectations of this position. Work is performed in an office environment and in the community.  Community outreach is typically done outside, so much be able to withstand all outside climate variables (e.g. heat, farmland, etc.). Must be able to work flexible hours, as evening and weekend work are necessary. Contact with staff and external clients and vendors. This position requires considerable driving to remote locations.

Duties/ Responsibilities – 

  • Performs outreaching tasks, such as camp screening, referrals, and follow up as needed.
  • Develops and maintains relationships with key individuals and agencies in the community.
  • Provides direct services: such as transportation, interpretation, food, clothing and toiletries.
  • Use health assessment to evaluate health risks and other social needs/follow ups.
  • Distributes information to farmworkers regarding clinic services, and other state/locals services.
  • Maintaining current information on camps/housing locations, demographics, and health status on farmworker communities, maintains outreach supplies.
  • Proper and accurate documentation on all patients seen within the program.
  • Contacts other health professionals or agencies if needed to provide optimal care for the patient.
  • Works with other health center staff or outside medical professionals to provide care for patients.
  • Provide case management services (such as assessment, referral, follow up and reassessment when needed).
  • Demonstrates the ability to work with diverse patient populations served and a commitment to advocacy for cultural diversity.
  • Reviews chart notes, past visits prior to meeting with patient.
  • Utilizes open-ended questions to obtain additional information.
  • Maintains a current outreach manual, community resource list, health education materials, etc
  • Approximately 70% of time spent outside clinic setting providing health education
  • Maintains outreach record-keeping system and assures accurate documentation on all forms/systems
  • Provides health education on a variety of required topics (green tobacco sickness, pesticide exposure, hypertension, high cholesterol, etc.)
  • Participates in outreach staff recruitment, orientation, supervision, and task/project/work scheduling
  • Completes health assessments with farmworkers at farm worker locatins using appropriate forms (Pediatric, Adolescent, Adults) enable encounter forms, and site registration
  • Participation in operation planning retreat annually
  • Submits request to Outreach Program Coordinator, related to supply needs
  • Participates in health center/clinic staff meeting to provide input
  • Functions as a liaison between outreach workers and the health center
  • Utilizes FHASES (Farmworker Health Administration Electronic Services)
  • Assists with feedback survey for farmworkers to NCFHP as specified by deadline
  • Assists in the collection of donations from churches, and other organizations, pack and distribute to farmworkers

Qualifications – 

  • Meet a standardized set of competencies for the specific position description established by PHSC and approved by CMS before working independently.
  • Must possess knowledge and skills necessary to employ the nursing process to perform care for patients.

Education: Minimum requirement is high school diploma, preferred associate or bachelor's degree or equivalent experience and/or equivalent training and/or experience.

Required: Bilingual English/Spanish required. This position requires considerable driving to remote locations.  A valid driver's license and proof of

insurance is required for this position.  Is expected to drive a company vehicle to transport patients as needed, so must maintain an acceptable driving record

  • Requires frequent sitting for long periods, operation of standard office machines and computer. 
  • May require lifting of up to 25 pounds. 
  • Requires hand-eye coordination and manual dexterity. 
  • Requires use of office equipment, such as computer terminals, telephones or copiers.
  • Requires normal vision range.

Preferred: Public health outreach preferred.

Current/valid License: Driver's License

Pay Range - $20.48- $27.52
