About Position: The primary responsibility of the Pharmacist is to assure the safe and appropriate use of drugs through the provision of drug information and drug therapy monitoring within the framework of SeniorCare pharmacy policy, standard practices and state and federal regulations.

Duties/ Responsibilities –
- Drug cost control.
- Management of polypharmacy.
- Provides improved participant are through drug information, drug therapy monitoring and participant/staff education.
- Assistance with pain management.
- Management of medications for patients with chronic conditions in an outpatient LTC setting.
- Conducts lab reviews, authorizes prescriptions, enters orders and refills, enters medication list for initial FL2.
- Demonstrates and maintains professional knowledge/technical skills and exhibits the ability to communicate this expertise to others. Participates in in-servicing new employees and continuing education for staff.
- Once a month, assists Pharmacy Administrator with review of PDE for clinical accuracy and assists in error reconciliation as needed.
- Acts as liaison between contract pharmacy and assigned area, solving distribution problems and monitoring drug supplies for proper storage, etc.

Qualifications –
- Individual therapy skills for geriatric participants
- Analytical thinking and problem solving abilities
- Ability to handle multiple complex priorities

- Problem solving skills
Education: B.S. Pharm or Pharm D from an accredited School of Pharmacy; ASHP accredited residency program.
Required: (Bilingual) 2 years professional level experience in a health related area.
- Requires frequent lifting and carrying items weighing up to 40 pounds unassisted.
- Requires frequent bending, reaching, and repetitive hand movements, standing, walking, squatting and sitting, with some heavy lifting, pushing and pulling exerted regularly throughout a regular work shift.
- Requires exposure to communicable diseases and/or bodily fluids.
- Hearing: Adequate to perform job duties in person and over the telephone.
- Speaking: Must be able to communicate clearly to medical providers, patients and multiple personnel in person and over the telephone.
- Vision: Visual acuity adequate to perform job duties, including reading information from printed sources and computer screens. Must meet certification by colorblind standards.
Preferred: One year of experience with the frail or elderly preferred.
Current/valid License: NC Registered Pharmacist License required, National Certification preferred; Valid NC driver's license.
Immunizations: Be medically cleared for communicable diseases and have all immunizations up-to-date prior to beginning employment.
Pay Range -$119,690 - $134,712
