About Position: The primary responsibility of the Social Worker is to provide social work care management services to the participants of the Piedmont Health SeniorCare program; provides services such as participant screening, assessments, care management, supportive listening and coaching and referrals.

- Schedule: Monday – Friday 8:00 AM – 5:00 PM

Duties/ Responsibilities –
- Ability to conduct a comprehensive biopsychosocial assessment.
- Participate in IDT initial assessments, care planning and periodic reassessments for participant's care. Communicate participant changes to team members.
- Coordinating home care services.
- Provides counseling to participants as indicated and incorporates into care planning. Utilizes critical thinking in clinical problem solving and decision making relative to cope of practice, knowledge, competency and experience.
- Facilitate family meetings. Chart the family meeting in Centricity and update the Care Team for that participant on outcomes from the meeting via morning meeting or e-mail.
- Knowledge about illnesses and medications and their psychosocial sequelae.
- In collaboration with Resource Coordinator, refers participants and families to appropriate community agencies or facilities, acts as liaison with such organizations and as an advocate for participants.
- Ability to educate professional staff and community providers regarding psychosocial factors and family dynamics impacting response to treatment.
- Serves as liaison between participants and families and the Interdisciplinary Team regarding psychosocial issues, family dynamics, and home situation factors that affect health and well-being.
- Communicate information about participants' experience in day program to IDT members and help resolve problems/issues in this setting.
- Care plan assessments in medical record.
- Assess safety and support at home for enrollment into PACE and on an on-going basis.
Qualifications –

Education: This position requires a master's degree from an accredited school of social work, 2 years professional level experience in a health related area.
Required: 2 years professional level experience in a health related area.
- Requires frequent lifting and carrying items weighing up to 40 pounds unassisted.
- Requires frequent bending, reaching, and repetitive hand movements, standing, walking, squatting and sitting, with some heavy lifting, pushing and pulling exerted regularly throughout a regular work shift.
- Requires exposure to communicable diseases and/or bodily fluids.
- Hearing: Adequate to perform job duties in person and over the telephone.
- Speaking: Must be able to communicate clearly to medical providers, patients and multiple personnel in person and over the telephone.
- Vision: Visual acuity adequate to perform job duties, including reading information from printed sources and computer screens. Must meet certification by colorblind standards.
Preferred: (Bilingual) One year of experience with the frail or elderly preferred.
Current/valid License: MSW or LCSW. If not a licensed clinical social worker (LCSW), the incumbent must obtain NC Certified Master Social Worker within sixty days of employment. Reference: http://www.ncswboard.org/page/certification-licensure.html Valid NC driver's license and a reliable vehicle.
Immunizations: Be medically cleared for communicable diseases and have all immunizations up-to-date prior to beginning employment.
