About Position: Working under the direct supervision of the Clinical Services Director, the primary responsibility of the RN Care Manager is to provide direct and indirect nursing care to assigned patients by utilizing critical thinking skills to assist in the management of chronic diseases, clinical problem solving and decision making in collaboration with the IDT. The RN Care Manager works independently in the application of advanced nursing knowledge and skills relative to NCBON scope of practice, competency and experience. Participates in call rotation, performs home visits, end of life/palliative (aka Pathways), and nursing care per nursing protocol. Completes professional performance requirements annually.

Duties/ Responsibilities –
- Complete initial, semi-annual, annual and change of status RN Assessments to evaluate health status and safety in the home for assigned patients. Assessments must be completed prior to scheduled IDT meetings. Completes assessments per PACE policy as required for patient and/or family request for services.
- Works independently to assess and evaluate patient or caregiver understanding of disease process, treatment plan and/or lifestyle changes to assist in the management of health care conditions, such as high blood pressure, diabetes, end of life, COPD and/or CHF. Serves as a resource to the entire team for the management of these patients and conditions.
- Completes and documents weekly visits to facility to check in on participant, monitor medication adherence and clinical care being provided.
- Review individualized care plans for assigned participants in a timely manner. Review to include obtaining participant or participant designee signature of care plan and standard PHSC immunization consent (as identified on care plan signature document).
- Participates in chart auditing processes as well as self-evaluates work to ensure compliance with agency regulations, policies and procedures.
- Ensures the completion of monthly vital signs, weights and skin/foot check completion for participants on assigned panel and reviews to determine if additional nursing intervention is needed.
- Requires frequent lifting and carrying items weighing up to 40 pounds unassisted.
- Requires frequent bending, reaching, and repetitive hand movements, standing, walking, squatting and sitting, with some heavy lifting, pushing and pulling exerted regularly throughout a regular work shift.
- Requires exposure to communicable diseases and/or bodily fluids.
Qualifications –
- Must possess knowledge and skills necessary to treat patients.
- Meet a standardized set of competencies for the specific position established by PHSC and approved by CMS before working independently.
- Excellent interpersonal, oral and written communication skills.
- Adequate to perform job duties in person and over the telephone.
- Must be able to communicate clearly to medical providers, patients and multiple personnel in person and over the telephone.
- Visual acuity adequate to perform job duties, including reading information from printed sources and computer screens. Must meet certification by colorblind standards.
Education: Graduate of an accredited school of nursing.
Required:
- RN currently licensed in NC
- 2 years of experience as a nurse.
- Present credentials which demonstrate ability to meet performance criteria for the RN position;
Preferred: Has practiced nursing in the last 3 years nursing experience. Prefer experience in home care or long-term care setting, ideally PACE. RN required. One year of experience with the frail or elderly preferred.

- Valid NC driver's license.
